cuda-gdb: failed to read the grid index

I am trying to get cuda-gdb to break when entering my kernel, but I am getting this error:

Breakpoint 2 at 0x103030c00: file foo.cuda, line 147.

Pending breakpoint 1 - "foo_kernel" resolved

What is causing this?

Which toolkit version are you using? Which driver version? What hardware?

CUDA 4.0.19 on OS X Lion Preview. Toolkit is 4.0.17.
