Hi, Dan - can you confirm that both the 6-pin and 8-pin connectors from the riser card are connected to the dongle cable from the Tesla M60? From the 2nd picture you posted, it looks like the 6-pin connector is not plugged it.
Cable should go:
/=> 6-pin <= 6-pin
Riser <=| |=> 8-pin => GPU
=> 8-pin <= 8-pin /
