Ubuntu 16.04.3 LTS, 4.4.0-109-generic #132-Ubuntu SMP Tue Jan 9 19:52:39 UTC 2018 x86_64 x86_64 x86_64 GNU/Linux, GeForce GT 430 nvidia driver 384.111

Xorg goes to 100% and stays there, GUI freezes. SSH from remote is ok.
Sometimes the machine is stable for days and then it happens. Other times it happens minutes after a fresh reboot.

nvidia-smi

Unable to determine the device handle for GPU 0000:01:00.0: GPU is lost.  Reboot the system to recover this GPU

NVRM: Xid (PCI:0000:01:00): 79, GPU has fallen off the bus.

Can happen due to overheating, flakey power source or a dying gpu.
Check temperatures using nvidia-smi, clean heatspreader and fans, try another PSU.
