GPU driver is lost when reboot the system

I install the driver NVIDIA-Linux-x86_64-450.66.run on Ubuntu 18.04. But SOMETIMES, the diver is lost after rebooting.

The output of nvidia-smi is:
NVIDIA-SMI has failed because it couldn’t communicate with the NVIDIA driver. Make sure that the latest NVIDIA driver is installed and running.

The output of lspci | grep -i nvidia is
