GTX 950 - Drivers or Damaged?

Any help that you can provide me with is greatly appreciated. I am at a loss as to what is going on
nvidia-bug-report.log.gz has been attached.

System Description
Embedded Headless System
Graphics Card: GTX 950
OS: Ubuntu 18.04
Drivers: nvidia-compute-utils-435, nvidia-dkms-435, nvidia-driver-435, nvidia-kernel-common-435, nvidia-kernel-source-435, nvidia-prime, nvidia-settings, nvidia-utils-435
Cooling: Fanless w/ Dual-sided aluminum heat sink

Graphics card was working prior to installation. Recently, it was noticed that the graphics card driver was not loading at boot. Further investigation shows the graphics card not being displayed with the lspci command.

For comparison, I have a 2nd identical system that is functioning correctly. This is what is displayed by the lspci command when run on the working system. Bold items are highlighting differences between nominal machine (below) and device under question (above)

As shown on the nominal system, the graphics card is attached to the pci bus via PCI Express Root Port (1c.0). However, the questionable system is not showing the pci bridge connected to the nvidia graphics card. The questionable system’s BIOS, is not detecting the nvidia graphics card so therefore it is defaulting to the integrated graphics card.

On my nominal system, I have completely deleted all nvidia drivers but lspci still detects the nvidia graphics card.

I have measured the ambient temperature several times and it was ~55C. The system is supposed to be rated up to 70C. I have not continually logged the temperature so I cannot say with confidence that 70C has not been exceeded

The gpu is gone. Please try reseating it in its slot, reseat power connectors, since the whole pci bridge went missing, reset cmos.

Any idea what might cause something like this to happen?

Overcurrent/short circuit? General mainboard damage? IDK, pure speculation.
