README for vulkan integration is wrong

the example snippet here:

        VkDeviceDiagnosticsConfigCreateInfoNV aftermathInfo = {};
        aftermathInfo.sType = VK_STRUCTURE_TYPE_DEVICE_QUEUE_CREATE_INFO;
        aftermathInfo.flags = aftermathFlags;

        // Set up device creation info.
        VkDeviceCreateInfo deviceInfo = {};
        deviceInfo.sType = VK_STRUCTURE_TYPE_DEVICE_QUEUE_CREATE_INFO;
        deviceInfo.pNext = &aftermath_info

incorrectly has
aftermathInfo.sType = VK_STRUCTURE_TYPE_DEVICE_QUEUE_CREATE_INFO; (should be aftermathInfo.sType = VK_STRUCTURE_TYPE_DEVICE_DIAGNOSTICS_CONFIG_CREATE_INFO_NV;

and also has deviceInfo.pNext = &aftermath_info; when it should be deviceInfo.pNext = &aftermathInfo;.

Also, I have a crash in nvoglv64.dll when using validation layers in my vulkan application, and the callback registered via GFSDK_Aftermath_EnableGpuCrashDumps is NOT being triggered, so I don’t know how to proceed in assisting to fix the underlying driver bug (nor even to work around it in my application).
