USB device can't be recognized on self-made carried board

Hi:
My USB device can’t be recognized on self-made carried board ,during entering system configuration and Ubuntu OS desktop.However,i can burn image by USB 2.0 port 0 that be designed to Micro USB.
This picture describes my USB’s hardware design.
Please help me to solve this problem,Thanks!
