Random black screen during FFXIV Heavensward gameplay

I have this PC and installation running for a couple years now while playing various games without any issue from the graphics card.

Recently I started FFXIV and so far everything worked fine, until I started to play the first expansion content. During game play I get at random times black screens. The system is still running, I just don’t get any video output anymore. I connected via SSH from another computer and watched “dmesg”. After the black screen occurs I see the following messages there:

I have no idea what is happening here. Is the GPU crashing?

According to this chart Xid 16 means HW error.

Not sure if I read that table correctly, but it looks to me like Xid 16 says “Display engine hung” and “Driver Error”

Is there an actual bug tracker for such things or is this is?
