`Failed to initialize NVML: Driver/library version mismatch` occurs from time to time

On ubuntu server.
Reinstall driver solves this issue but after several non-issue reboots and weeks of usage, this problem always re-emerge.
There had been no new package install or upgrades done manually. (Except for, on another os on the same system, windows. which I don’t think would interfere)

I am not sure what else could possibly cause this. Is there a way to stop this from happening again?

My first suggestion (without checking the log yet) would be to disable automatic updates of third party drivers in Ubuntu.

Ubuntu has a nasty habbit of running “behind-your-back” updates in case it deems them essential and those can on occasion disrupt the NVIDIA GPU driver.

Maybe that helps already.
