In R32.4.2 Xavier,I use sudo apt-get upgrade to update to R.32.4.3. I custom the dtb, and replace the kernel/dtb/tegra194-p2888-0001-p2822-0000.dtb, then I use the following command to update:
$sudo ./flash.sh -r -k kernel-dtb jetson-xavier mmcblk0p1
host show kernel-dtb has been updated successfully, the xavier reboot, but dtb not my custom dtb?
Have you checked the device tree in /proc/device-tree? The flash process does not change the .dtb file in /boot.
@waldman, Yes,I checked the device-tree,it is not changed. I follow the procedure in CBoot doc to change /boot/extlinux/extlinux.conf, now I can use my own dtb.
Before I upgrade the system, flash.sh method is always valid. So, the upgrade bring the A/B boot question.

please refer to CBoot session and check [Kernel Boot Sequence Using extlinux.conf].
you may also review your extlinux.conf
configuration files, it’s by default loads the device tree from kernel-dtb partition without FDT entry.
