nvidia-smi reports phantom utilization reported on one GPU

Immediately after boot. System is idle. Utilization on GPU 2 persists.

If a fourth GPU is installed, the new GPU which is designated as GPU 3 reports a phantom utilization.

OS is RedHat 6.9. This driver is 390.46, but I have seen similar behavior with other driver version too.
