btw, what power mode are you using now?

HW throttling (OC alarm) would happen when input voltage drops below safe operating voltage ~4.5V. Please make sure you have a stable input 5V@4A power supply.

Thanks. I am powering from a 10A benchtop supply, but I do see some transients on the 5V rail. I will add more bulk capacitance to the board.
