Make sure no stray blacklist files remain:
sudo rm /lib/modprobe.d/blacklist-nvidia.conf /etc/modprobe.d/blacklist-nvidia.conf
sudo update-initramfs -u
then reboot and create a new nvidia-bug-report.log.
