Thanks @generix for helping out. By looking at the logfile of the bug report I saw that the wrong kernel was still selected. When changing to 5.11.0.41 (generic) it works!

I am now experiencing the same problem (nvidia-smi returns an error) after upgrading my Ubuntu from 18.04 to 20.04.3.
Also, because of this problem, I suspected my OS cannot connect to the external monitor.
Some information about the graphic card of my PC, I have a hybrid graphic card - Intel and NVIDIA
I have tried the following to solve the problem, and they did not solve the error:

Remove all nvidia packages, and re-install Nvidia package/driver by using sudo ubuntu-drivers autoinstall

Install different Nvidia drivers (390, 460, 470, 495)

Upgrade my kernel from 5.7 to 5.11, remove all nvidia packages, re-install the package
-
Install nvidia cuda toolkit
-
Disable Secure Boot
Would you be so kind to help me? I have been banging my head to solve this problem without luck.

The nvidia gpu is simply turned off. Due to driver flooding the logs because of this, I canāt see the reason. Please check if you have bbswitch installed and uninstall it.

Iām experiencing the same issues as everyone in this post, even after following all the suggestions. Iām currently running the 495.29.05 drivers for an GTX 765M.
Things checked:

secure boot is disabled
-
no blacklist-nvidia.conf file exists
-
no multiple versions of drivers installed

I guess that says it all⦠Install the v470 driver (through Software & Updates)⦠(remove v495 first of course).
I have been trying to figure out the issues for days now and thanks to your suggestion in running the nvidia-bug-report.sh (Iām new to linux) I was able to at least have a clue for why this is happening.
The thing that caught my attention was Unable to find installed file '/etc/OpenCL/vendors/nvidia.icd' (No such file or directory).
Iām also attaching the log file and would really appreciate any advice.
I have searched for a solution but nothing I tried worked
I installed the driver after running the command sudo ubuntu-drivers devices
and installed nvidia-driver-495
since it had recommended next to it.

@generix I have upgraded to Ubuntu 21.10 and using the latest Kernel also checked all the solutions listed above but without any results.

In your previous bug-report.log you had a kernel built for 21.10 installed in ubuntu 20.04 which prevented the nvidia mudule to build.
Your current bug-report is stuck in the virus scanner. Please open a new thread, since as a new user, you can only reply three times to a foreign thread.

- no blacklist-nvidia.conf file
- nvidia-prime: nvidia profile already set

You have the wrong gcc version set as system compiler, should be 7.5 but is 6.5.

So i recently am experiencing the same problem. I noticed, when my second monitor stopped working and my first monitor,s resolution was wrong and couldnāt change it.
- Prime Profile Already selected
- No blacklist-nvidia.conf
- No nvidia entry in /etc/modprobe.d/blacklist.conf
- Secure boot OFF
- Nvidia Settings says the Driver isnāt loaded
What i have tried to resolve the Issue:
- Reinstall Drivers via āAdditional Driversā: Nothing happened
- Install Official Nvidia Driver: Threw GCC one time, but hasnāt worked since.
- Fixing GCC by installing the correct Version: Still no luck
- Installing Drivers from System76
- Deleting X11 Config file, and monitors.xml
- Tried different Drivers.
GPU: GeForce RTX 3060 TI
CPU: Ryzen 9 3900XT
OS: Zorin OS 16 (Fork of Ubuntu 20.04 Focal)

You built the driver for kernel 5.16.
Kernel command line: BOOT_IMAGE=/boot/vmlinuz-5.11.0-44-generic
But all the boot entries in the log are from kernel 5.11.
So either boot the 5.16 kernel, or install the nvidia driver for the 5.11 kernel using dkms:
sudo dkms install -m nvidia -v 470.86 -k 5.11.0-44-generic

There doesnāt seem to be any kernel modules installed. Please reinstall the kernel headers
sudo apt install --reinstall linux-headers-$(uname -r)
and post the output of
dkms status
